    1. Base Rate: $82
    2. Audatex Fee: $6 (Bill as ancillary charge. IT is NOT INCLUDED)
    3. ESV: $22 (Bill as ancillary charge. IT is NOT INCLUDED)
  3. Mileage
    1. Standard – $1.20/mile
    2. Long Distance Pricing (> 101 mile) – $.74/mile
  4. Time & Expense (T&E) – Market Rate
  5. CAT Pricing – Based on Storm
